Fulham striker Aleksandar Mitrovic was sent off for barging referee Chris Kavanagh in a moment of madness against Manchester United.
The Cottagers lead 1-0 when Jadon Sancho rounded Bernd Leno and looked set to put the hosts level before Willian blocked his effort off the line.
Kavanagh went to the VAR screen to check for a possible handball before sending off Fulham boss Marco Silva for dissent.
Willian was also sent off for denying a goalscoring and Mitrovic reacted angrily as he shouted at the referee before barging him.
The Serbian was shown an immediate red to leave Fulham down to nine men and without their manager on the touchline.
Mitrovic could face a lengthy ban in a blow to the west London side. Former West Ham forward Paolo Di Canio was banned for 11 games for pushing the referee in similar fashion in 1998.
Bruno Fernandes converted from the penalty spot to put United level in the 75th minute, having sent Bernd Leno the wrong way.
Marcel Sabitzer put Erik ten Hag’s side ahead two minutes later with a close-range finish from Luke Shaw’s finish.
Fernandes got his second of the game and sealed the win in the six minute of additional to book a semi-final spot.
